PURPOSE: The purpose of this form is to provide U.S. citizens with the ability to submit general inquiries to the Department of State unrelated to passports, overseas citizen services, visas, adoptions, emergency assistance or the resource topics provided within the form.
ROUTINE USES: The information on this form will be used to forward the inquiry to the appropriate Department program office. This information will not be shared outside of the Department of State.
